What is Adult Family Care (AFC)?
Adult Family Care (AFC) provides clinical and financial supports to qualifying caregivers or family members, assisting them in caring for the elderly and adults with developmental disabilities in a home setting.

Eligibility Criteria Made Easy
Determining if you qualify for Adult Family Care benefits doesn’t have to be a complex process.
To be eligible for the Adult Family Care program, you must meet the following eligibility criteria.
Caregivers:
- Caring for someone with a medical, physical, or developmental disability that prevents them from living independently, and needs supervision or assistance with at least one basic task of everyday life (e.g., eating, bathing, etc.) This can include someone living with dementia as well.
- Caring for someone who is at least 16 years of age or older.
- Can be the parent, family member, or nonrelative of the person being cared for.
Exclusions:
- Cannot be the legal guardian or spouse of the person being cared for.
- Must be a resident of Massachusetts.
- Caregiver must be at least 18 years of age.
- Must be able to pass a Criminal Offender Record Information check.

What is the Adult Family Care program?
The Adult Family Care (AFC) program provides clinical and financial assistance to qualifying caregivers or family members to assist them to care for their loved ones with disabilities in a home setting.
How is the Adult Family Care program funded?
The Incompass AFC program is funded by MassHealth through the Executive Office of Elder Affairs. This program offers MassHealth-eligible individuals over the age of 16 the option to continue living at home with their families or with non-family members.
Who is eligible for support?
Individuals, 16 years of age and older, on MassHealth who are unable to live alone.
Who is eligible to apply?
Qualified caregivers are able to meet the daily care needs of participants and follow program guidelines. A caregiver may be a family friend or family of the participant, except a spouse or legal guardian, and must live in the same home as the participant. You may be eligible if your loved one requires assistance with bathing, mobility, dressing, toileting, and other elements of personal care. I’m a parent. Am I eligible to be a caregiver?
Per MassHealth, you are not eligible if you are a parent of a minor child who would be the AFC individual; the spouse of the individual; or a legally responsible relative, such as a legal guardian of the individual.
If approved for the program, what services can I expect?
An Incompass care manager will teach you how to manage everyday challenges to give you more time to enjoy the everyday triumphs. You will learn about your community resources, caregiver compensation, relief, and alternative placement, respite services, case management, and support, nursing support, intake assessment, and 24-hour emergency care consultation.
Can I apply to be a caregiver if I am not related?
Incompass AFC caregiver placements include several successful nonbiological matches. If you would like to be a caregiver for someone in your own home, or if you don’t have a family caregiver but need assistance, call AFC to set up an exploratory interview, at 978-349-3023.
